🔬 The VITAL Trial: Do Vitamin D and Omega-3 Affect Cancer and Heart Disease
25,871 participants, 5+ years of follow-up — what one of the largest prevention trials showed
VITAL (VITamin D and OmegA-3 TriaL) is a large randomized placebo-controlled trial that tested whether vitamin D and Omega-3 reduce the risk of cancer and cardiovascular disease in healthy people. It was conducted at Brigham and Women's Hospital and Harvard Medical School and was one of the most anticipated trials in nutrition science.
Before VITAL, most data on vitamin D and Omega-3 came from observational studies, which cannot prove cause and effect. VITAL was designed to answer whether these supplements work in real-world prevention among "ordinary" people without obvious deficiencies.
What is the VITAL trial
VITAL is a randomized double-blind placebo-controlled trial launched in 2011. Its main goal was to assess whether taking vitamin D and/or Omega-3 could reduce the risk of cancer and major cardiovascular events in healthy adults.
Design: who participated and what they took
The trial enrolled 25,871 people: 13,085 men over 50 and 12,786 women over 55. Participants had no history of cancer or cardiovascular disease at enrollment, so the trial was truly preventive.

Cancer results
The overall rate of invasive cancer did not differ between the vitamin D and placebo groups. That is, vitamin D did not reduce the risk of developing cancer overall in this population.

Cardiovascular results

Additional and secondary data
VITAL produced many secondary publications — on cognitive function, depression, diabetes, infections and other outcomes. In almost all of them, vitamin D and Omega-3 showed no significant effect on secondary outcomes in the general population.
Limitations of the trial
To interpret VITAL correctly, it is important to understand its limitations:
Moderate doses
2000 IU of vitamin D and 1 g of Omega-3 are preventive, not therapeutic, doses. Results do not rule out effects at other doses.
Healthy people
Participants had no cancer, heart disease or, in general, marked deficiencies. The data do not transfer directly to people with deficiencies.
Limited time
The median follow-up was ~5 years. For cancer and atherosclerosis this is relatively short; an effect might emerge later.
Subgroups
Subgroup findings are secondary analyses, prone to chance findings, and need confirmation.
Practical conclusions
How to use the VITAL results in practice:
Check your status, don't "prevent blindly"
Before taking vitamin D, check your 25(OH)D level. Supplements are justified mainly for deficiency, not "just in case".
Omega-3: consider fish intake
The heart attack signals were stronger in people with low fish intake. Fish in the diet is a sensible goal for many.
Don't expect "miracles" from supplements
Healthy eating, movement, quitting smoking and blood pressure control affect cancer and heart risk more than any supplement.
Discuss with your doctor
Individual decisions (especially with deficiencies or high risk) are best made with a doctor based on tests.
